Consulting Palliative Care
After a complete assessment of your hospital’s resources, needs and priorities, a custom package will be designed for you. This could include some or all of the following:
- A Palliative Care Consult Service or In-patient Unit
- A clinical pathway to guide healthcare providers as they attend to the dying patient and his or her family
- End-of-Life education for nurses and allied healthcare
- Train the trainer programs for hospital educators in end-of-life care
- A “Resolve Thru Sharing” program to address perinatal loss and death
- Programs to address “Compassion Fatigue” in healthcare workers dealing with patients at the end- of-life
- Development of relationships with local hospices and palliative care home health programs or development of palliative care and hospice in your existing home health agency
- Grand rounds and in-service education on related topics
- Advance Care Planning training
- Develop and monitor quality improvement measures
