Internists are responsible for providing a high level of quality, evidence-based health care to the patients of the health center, including patients of moderate-to-high complexity in their medical needs. The primary care physician fosters a patient-centered relationship and ensures that patients are appropriately screened, diagnosed, and medically managed for their acute or chronic conditions. Each medical providers collaboratively participates in a multidisciplinary team that includes mid-level providers, mental health providers, nurses, medical assistants, case managers, and peer navigators.
- Perform patient history and physical, including pelvic examinations
- Order appropriate diagnostic testing, including but not limited to laboratory tests, pathology, physiological studies, and imaging. Apply expert knowledge in the interpretation of results to management of patient conditions
- Develop a patient-centered plan of care in collaboration with care team
- Prescribe medications as in accordance with professional standards of care and state and federal prescribing guidelines
- Make appropriate referrals to internal or external providers for services
- Provide counseling and education to patient for preventative, acute, and chronic disease care
- Be familiar with and adhere to the Clinic Policies and Procedures relevant to job duties, including following infection control guidelines
- Assess and thoroughly document findings and treatments in patients’ electronic health records
- Perform all duties for successful EHR/EPM including completion of medical records after each visit, completion of documentation, coding, and signing off charts within the designated time set by the organization.
- Provide supervision in care management and procedures to NPs and PAs as needed including sign-off on charts, co-signatures for forms and prescriptions
- Perform or supervise support staff on standard clinical procedures, including but not limited to venipuncture, IM & Sub Q injections, EKG control of external hemorrhage, application of dressings, I&Ds, wound care, sutures, collection of specimens for culture, laboratory testing
- Consult with the Chief Medical Officer as needed on clinical matters
- Travel from one health center location to another for the provision of medical care
- Maintain the confidentiality of all patients and their family in accordance with local, state, and federal privacy laws
- From time to time, provide in-service training for staff in selected topics within scope of practice of medicine as appropriate, and as deemed necessary by the CMO or his/her designee
- Work with the support staff to schedule their time at the Clinic at least 3 months in advance. Provide dates and times of potential conflicts within the timelines required of all staff in accordance with Personnel Policy and Procedures.
- Work collaboratively to bring the clinic to the quality standards of a Patient Centered Medical Home (PCMH), Federally Qualified Health Center, and other required standards
- Participate in continuous quality improvement measures including evaluation of clinical treatment, clinical outcomes, and practice benchmarks
- Attend internal and external meetings, workshops, and conferences, and participates on task forces or committees, as necessary and/or required by the organization.
- Contribute to the financial sustainability of the health center by way of, but not limited to, accurate billing, patient census growth, community partnership building, developing public insurance participation and coverage, and following & implementing health networks’ operational plans
- Responsible for advising the Chief Medical Officer of any change in status that would affect their ability to perform services
- Maintain fund of knowledge through ongoing CME
- Other duties as assigned
- MD or DO degree from a recognized and accredited University
- Unrestricted, active license to practice medicine in the State of California
- Possession of current DEA certificate
- Active board certification with relevant national professional board in designated primary care specialty field of practice (eg, ABIM)
- Active BLS certification or equivalent certificate
- Extensive knowledge of the principles and practice of general internal medicine
- Knowledge of diagnostic tests and findings and therapeutic procedures as they relate to family practice
- Knowledge of effective infection control measures
- Excellent problem solving skills required, including creativity, resourcefulness, timeliness, and technical knowledge related to analyzing and resolving medical problems.
- Ability to establish and maintain effective working relationships with patients, staff, and physicians
- Strong verbal and written communication skills
- Experience working in an interprofessional, team-based environment
- Proficiency in Microsoft Word, Excel, and Outlook, and at least one EHR system
- High level of cultural competency working with marginalized populations including the LGBTQ community, racial/ethnic minorities, substance-using individuals, and low-income/homeless
- Experience prescribing hormones for gender reassignment
- DEA waiver for prescribing office-based opiate substitution therapy (e.g., buprenorphine)
- Certification with the American Academy of HIV Medicine or equivalent experience treating with people living with HIV
- Proven clinical skills and experience with health care and social service systems in San Francisco
- Fluency in English and one or more of the following: Spanish, Tagalog, Thai, Vietnamese, Mandarin, or Cantonese.
